【麻痺側上肢の自主トレ目標】③ Step by Step・座位姿勢を整える・麻痺手のトレーニングを効果的に行うための準備と姿勢
Even if paralyzed limbs appear completely immobile, in many cases the ability to move them still exists (latent movement). The goal of rehabilitation is to improve the condition where this ability cannot be effectively utilized. In rehabilitation, a key aspect is how to alleviate muscle stiffness (spasticity), a common issue for many patients, and prevent it from hindering movement. Forcing muscle training while maintaining poor posture can lead to compensatory movements (excessive force in the body), ultimately worsening stiffness (spasticity) on the paralyzed side. This is one reason why muscle training can cause stiffness. The spine and scapula alignment: When back muscles such as the latissimus dorsi become stiff, the body tilts and collapses towards the paralyzed side. Correcting this "collapsed state" is paramount. As a prerequisite for muscle training, the goal is to be able to maintain good posture without conscious effort. ■ Practical method using a towel 1. Place a thickly rolled towel under the buttocks on the paralyzed side and wait a short time. 2. Find a position where your weight is evenly distributed between the non-paralyzed and paralyzed sides, your spine is straight, and your shoulder blades are at the same height. 3. Wait for sensation: Instead of "forcing it to correct," relax and wait for the "feeling of support from your buttocks and soles of your feet" to return. ■ Advice (Answer to Taaman's question) ・Regarding the request to do muscle training because the forearms have become thinner, it is OK to do so if your posture is correct. The key to muscle training on the paralyzed side is to use light force, not a straining "Hmm!" kind of force, but just enough to feel "Ah, my hand is moving a little." 1:43 How to control spasticity 2:14 Question from Taaman: Is it OK to do muscle training on my forearms that have become thinner? 3:05 Why stiffness in the back (latissimus dorsi) hinders movement of the paralyzed hand 4:20 The relationship between poor posture and the scapula/spine 5:05 Practice: How to correct sitting posture using a towel 6:31 The absolute requirement for starting muscle training: "Good posture without tension" 7:52 The trick to waiting for the "body's natural reaction" rather than conscious force 9:03 Sitting with weight on the buttocks of the paralyzed side ------------------------------------------------ ■Rehabilitation Studio WillLabo A private rehabilitation facility located in Ryogoku.
