There's a question many patients ask themselves when back pain persists: has my back pain become chronic, and what does that really change? Behind these words, there are often many unanswered questions. We often associate acute pain with something temporary, and chronic pain with something serious or irreversible. In reality, the difference is more subtle, and above all, much more reassuring than we imagine. Acute back pain is recent. It appears after unusual exertion, a sudden movement, or a period of overuse. It acts as a warning: the body signals that it is less tolerant of certain stresses at that moment. In most cases, this pain gradually decreases over time, as the back regains its ability to adapt. Chronic pain, on the other hand, is defined primarily by its duration. When pain persists for several months, we speak of chronicity. This doesn't mean the back is damaged or that the situation is worsening. It primarily means that the pain system functions differently. When pain persists, the nervous system becomes more vigilant. Signals that would have gone unnoticed before are perceived as painful. The pain threshold decreases. Pain becomes less linked to a specific movement and more influenced by context, fatigue, or stress. Over time, movement patterns also change. Out of fear of pain, certain movements are avoided or performed in a very controlled manner. This strategy, logical at first, gradually reduces the back's tolerance. Neuromotor control and proprioception become less effective at distributing stress, thus perpetuating sensitivity. An important point to understand is that medical imaging doesn't always explain the difference between acute and chronic pain. Normal images can be obtained with persistent pain, or abnormalities can be visible without pain. Therefore, the duration of pain does not necessarily reflect structural deterioration. The good news is that chronic pain isn't inevitable. It simply requires a different approach: less prolonged rest, more gradual progression, more appropriate movement, and reassurance. Improvement is often slower and fluctuates, but it is possible. Thank you for taking the time to watch this video.
