In this talk, I focus on an often-overlooked role of progesterone: brain health. Progesterone is far more than a hormone for the uterine lining. It is a powerful neuroprotective hormone that affects inflammation, myelin formation, neuronal repair, sleep, mood, and cognitive function. I explain why the way progesterone is commonly prescribed in menopause (daily oral dosing) can be problematic for the brain. When progesterone is taken orally, most of it is converted in the liver into the neurosteroid allopregnanolone, leading to high levels that can impair memory and overstimulate the GABA-A receptor in a drug-like way. I contrast this with physiologic progesterone replacement, given cyclically and via non-oral routes, which allows the brain to convert progesterone into allopregnanolone only as needed. This approach supports natural sleep, reduces neuroinflammation, protects neurons, and aligns with how the female brain evolved to function.
