Pharyngitis is the inflammation of the pharyngeal tissue; it is characterized by the triad of: sore throat (odynophagia), fever, and inflammation of the pharyngeal mucosa. It can be caused primarily by viruses and bacteria. One of the most common causes of pharyngitis in children under 15 years old is Streptococcus pyogenes pharyngitis, which produces bacterial pharyngitis that can be accompanied by pharyngeal exudate, scarlet fever, general malaise, abdominal pain, and other symptoms. Key points (taken from Harrison's Principles of Internal Medicine) ACUTE PHARYNGITIS • Etiology: The most frequent identifiable cause is respiratory viruses, but in approximately 30% of cases, it is not possible to identify the cause. Viruses: Rhinoviruses and coronaviruses cause approximately 20% and 5% of cases, respectively; influenza, parainfluenza, coxsackievirus, HSV, EBV, and HIV are other important viral causes. - Bacteria: Group A streptococcus (GAS) causes 5% to 15% of cases in adults. Fusobacterium necrophorum is increasingly identified as a cause of pharyngitis in adolescents and is isolated almost as frequently as GAS. Other bacterial causes include group C and G streptococci, Neisseria gonorrhoeae, Corynebacterium diphtheriae, and anaerobic bacteria. • Clinical manifestations: specific signs and symptoms sometimes suggest that one cause is more likely than another. – Respiratory viruses: symptoms are usually mild and accompanied by coryza without fever, painful cervical lymphadenopathy, or pharyngeal exudates.
