Distension activates visceral afferent pathways (visceral pain). 4. A vague, dull, and diffuse pain occurs in the midabdomen or lower epigastrium. 5. Distension increases, and multiplication of resident bacteria in the appendix causes reflex nausea and vomiting. 6. As pressure in the organ increases, venous pressure is exceeded. 7. Engorgement and vascular congestion occur. 8. The inflammatory process affects the serosa of the appendix and, in turn, the parietal peritoneum. This causes the characteristic shift of pain toward the right iliac fossa. 9. Lack of blood supply damages the walls of the appendix to the point of infarction and facilitates bacterial proliferation, which can lead to appendix perforation.
