Although most people don't realize it, the elbow plays a very valuable role in our daily lives. Without it, we would have great difficulty eating, bathing, driving, etc. Most of the time, we only realize its importance when we have an injury or even an elbow fracture. Three bones make up this joint: the humerus, radius and ulna. The humerus is the largest bone in the upper limbs, connected to the radius and ulna. An elbow fracture can occur in isolation in any of the bones or even in all of them together. In our text today, the subject is precisely that: elbow fractures. Main causes of elbow fractures The most common causes of elbow fractures are falls while walking, going up or down stairs, car or motorcycle accidents and during sports activities. However, any type of direct impact (blows) or indirect impact (that overloads the elbow joint) has the potential to cause fractures. In injuries caused by falls in which the person rests their hands on the ground and keeps their arm stretched out, the fracture usually occurs in the head of the radius, since the forearm receives most of the impact. Falls on the back, in which the forearm is supported on the ground to cushion the impact, favor the fracture of the proximal ulna, also called the olecranon. Treatment and need for surgery Fractures of the head of the radius often do not require surgery and can be treated with immobilization using a sling, plaster splint or orthosis, in addition to physiotherapy sessions. This evaluation, however, must be performed by a qualified orthopedist, supported by imaging tests, such as X-rays and tomography. In the event of a fracture of the proximal ulna or distal humerus, that is, the portion of the humerus furthest from the chest, surgical intervention is usually necessary. Therefore, immobilization for a long period could permanently compromise movement in that region, generating more problems than solutions. The surgical procedure generally varies according to the type of injury suffered. There is the so-called terrible triad of the elbow (which consists of a fracture of the head of the radius, the coronoid process of the ulna and dislocation of the elbow), transolecranon fracture-dislocation, Monteggia fracture, among others. In intra-articular fractures of the distal humerus, for example, the surgery consists of fixing the fractured bone with the help of two metal plates and screws. When the fracture generates many fragments, making it impossible to fix the plates, the surgeon usually chooses to perform arthroplasty, replacing the cartilage with a metal implant. This procedure, however, is generally only indicated for elderly patients (over 70 years old) who suffer from osteopenia. Post-operative and recovery Once the procedure is finished, it is ideal to start orthopedic physiotherapy treatment as soon as possible, to avoid stiffness in the elbow. Light stretching and the use of dynamic orthoses are great allies in the process of recovering flexibility. ARTHROPLASTY OF THE HEAD OF THE RADIUS (PROSTHESIS) A radial head prosthesis is commonly indicated when there is a comminuted fracture of the head of the radius, that is, in many pieces, and its reconstruction is not possible. We usually recommend it when there is also a history of associated ligament injury. More rarely, this prosthesis may be indicated in cases of deformity of this bone (sequelae of a fracture) or wear of the Capitular-Radial joint (when the involvement is almost exclusively of the Radial head - very rare). The approach for performing the prosthesis is through a lateral access route and the main care is to not injure the lateral ulnar collateral ligament, otherwise the patient may develop posterolateral instability.
