Without it, we would have enormous difficulty eating, bathing, driving, etc. Often, we only realize its importance when we suffer an injury or even an elbow fracture. Three bones make up this joint: the humerus, radius, and ulna. The humerus is the largest bone in the upper limbs, connected to the radius and ulna. An elbow fracture can occur in any one of these bones alone or even in all of them together. In today's text, we'll cover precisely that: elbow fractures. Main Causes of Elbow Fractures The most common causes of elbow fractures are falls while walking, climbing or descending stairs, car or motorcycle accidents, and sports activities. However, any type of direct impact (blows) or indirect impact (which overloads the elbow joint) has the potential to cause fractures. In injuries caused by falls where the person rests their hands on the ground and keeps their arm straight, the fracture usually occurs in the radial head, since the forearm receives the majority of the impact. Falls on the back, where the forearm is supported by the ground to cushion the impact, favor fractures of the proximal ulna, also called the olecranon. Treatment and Need for Surgery Radial head fractures often do not require surgery and can be treated with immobilization using a sling, cast, or orthosis, as well as physical therapy sessions. This evaluation, however, must be performed by a qualified orthopedist, supported by imaging tests such as X-rays and CT scans. In cases of fractures of the proximal ulna or distal humerus—the portion of the humerus furthest from the chest—surgery is usually necessary. Therefore, prolonged immobilization could permanently compromise movement in that region, creating more problems than solutions. The surgical procedure generally varies according to the type of injury. There is the so-called terrible triad of the elbow (which consists of a fracture of the radial head, the coronoid process of the ulna, and elbow dislocation), transolecranon fracture-dislocation, and Monteggia fracture, among others. In intra-articular fractures of the distal humerus, for example, surgery consists of fixing the fractured bone with two metal plates and screws. When the fracture produces many fragments, making it impossible to fix the plates, the surgeon usually opts for arthroplasty, replacing the cartilage with a metal implant. This procedure, however, is generally only indicated for elderly patients (over 70 years old) who suffer from osteopenia. Post-operative and recovery Once the procedure is complete, orthopedic physical therapy should ideally begin as soon as possible to prevent elbow stiffness. Gentle stretching and the use of dynamic orthoses are great aids in the process of restoring flexibility.
