Understanding the Shoulder Joint and Scapular Movements Properly The shoulder joint is a multi-axial joint with the largest range of motion in the human body. The reason we can move our arms freely is not simply due to the humerus, but thanks to the shoulder complex where the scapula and rib cage move together. ✅ Major Movements of the Shoulder Joint Flexion Extension Hyperextension Abduction Adduction External Rotation Internal Rotation Circumduction Why the Scapula Is Important The scapula glides over the rib cage, extending arm movement and determining shoulder stability. ✅ 6 Functional Movements of the Scapula Elevation Depression Protraction Retraction Upward Rotation Downward Rotation And in the sagittal plane, Anterior Tilt Posterior Tilt occur. The Most Confused Concept Elevation ≠ Upward Rotation Elevation is a movement where the entire scapula moves upward. The upper trapezius and levator scapulae are primarily responsible. On the other hand, upward rotation is a movement where the scapula rotates to create space to raise the arm. The serratus anterior, lower trapezius, and upper trapezius work together. In other words, ✔ Elevation = Moving upward ✔ Upward Rotation = Opening upward The Reason for Shoulder Impingement Upward rotation of the scapula is absolutely necessary when lifting the arm more than 90 degrees. Insufficient upward rotation leads to: reduced subacromial space rotator cuff compression shoulder impingement syndrome and reduced range of motion. Modern individuals often exhibit a dominant downward rotation pattern due to serratus anterior weakness pectoralis minor shortening and levator scapulae hypertonicity. Is downward rotation a bad movement? Absolutely not. Downward rotation is essential for pull-ups rows climbing and movements utilizing the latissimus dorsi. The problem is not downward rotation itself, but ❌ when downward rotation becomes dominant in a state of insufficient upward rotation.
