Free → https://pilotdebrief.com/pattern On an October morning in 2018, a Piper Seneca II departed Connecticut for South Carolina with a known-bad directional gyro, an overcast layer along the entire route, and an instructor who wasn't instrument current. The Seneca lifted off from Danbury with a flight instructor, a private pilot, and a passenger on board, bound for Charleston. The instructor told ATC he was climbing to 8,500 feet VFR — but the airplane never stopped climbing. Within minutes he was reporting an unreliable attitude indicator and admitting the airplane was in and out of IMC. The controller declared an emergency on the pilot's behalf and offered vectors back to visual conditions. Instead, the unpressurized twin kept climbing — past 16,000 feet, past 18,000, eventually to 19,400 feet — without any evidence of supplemental oxygen on board. Another pilot who had flown the airplane recently confirmed the directional gyro was already known to be unreliable, and a review of the instructor's logbook indicated he was not instrument current. Radar captured what came next: a series of figure-eight turns followed by a steep, descending spiral. A witness on the ground heard the airplane maneuvering above the overcast, then a pop, and watched large pieces fall through the clouds. The NTSB determined the instructor's decision to press a VFR flight into IMC with a known flight instrument anomaly led to spatial disorientation and loss of control, with his lack of recent instrument experience and degraded performance from hypoxia cited as contributing factors. The wreckage distribution was consistent with in-flight breakup from overstress during the uncontrolled descent. The pattern here is the stack — none of these holes were hidden. The weather was forecast. The directional gyro was a known squawk. The instrument currency was a logbook check. The oxygen requirement above 12,500 feet is a regulation, not a judgment call.
