Free → https://pilotdebrief.com/pattern On a December evening in 2019, a Cessna 182P on approach to Carlsbad, California, began throwing flash explosions from the cowling and filled the cabin with the smell of oil — moments before the engine quit entirely above one of Southern California's busiest interstates. The flight was minutes from McClellan-Palomar Airport when the engine started running rough. Troubleshooting did nothing. The roughness escalated into flash explosions visible from the left side of the cowling, oil smell flooded the cabin, and then the engine lost total power — leaving the private pilot with no runway and very little altitude. The pilot put the airplane down on southbound Interstate 5. The landing was hard enough to collapse the nose gear and buckle the aft fuselage, but he and his passenger climbed out. The airframe took substantial damage; the people inside did not. The postaccident teardown told a deeper story. The Nos. 5 and 6 connecting rods had separated from the crankshaft, and the crankshaft itself broke into three pieces. Metallurgical analysis found beach marks consistent with fatigue cracking, with the fracture initiating below the nitride layer near an oxide inclusion. The engine had been converted from the original Continental O-470-U to a PPA O-470-50 under an STC — but investigators also found a supercharger bolted to the engine that was not part of that STC, with no logbook entry documenting the installation. The NTSB concluded the crankshaft failed in fatigue, with the undocumented supercharger contributing additional stress that drove the failure. The pattern here isn't really about one cracked crankshaft. It's about what gets bolted onto an airplane outside the paper trail. STCs exist because the engineering behind them accounts for stress, loads, and limits.
