Seminar: Ten Clinical Rules for Beginners Chapter 54: Rule 10 - "The Relationship Between Diagnosis and Treatment Is Diverse" (Part 2 - A Method for Talking with Patients) By Dr. José María Álvarez - Department of Psychiatry and Mental Health, Río Hortega University Hospital, Valladolid, Spain Date: June 3, 2026 In this class, we will continue examining the relationship between diagnosis and treatment, focusing particularly on some of the principles that guide psychoanalytically inspired psychotherapeutic practice. My aim is to show that the effectiveness of a treatment depends not only on the technique used, but also on factors that are more difficult to quantify, such as transference, the therapist's position, listening skills, and how a working relationship is established with the patient. Drawing on everyday clinical experience, we will study fundamental issues such as preliminary interviews, clinical assessment, subjective localization, and the role of words, silence, and listening in the therapeutic process. We will also see how the clinician attempts to move from mere complaint to subjective involvement, helping the patient recognize their participation in what they are lamenting. This transformation constitutes one of the decisive moments in any psychotherapeutic work. Finally, we will pay special attention to the concept of transference, considered by Freud and later by Lacan as one of the pillars of the analytic experience.
