[Niigata] Super J Niigata aired on September 18th ------------------ This is the second installment of our special feature following the summer residents spent in Niigata. The setting is Murakami General Hospital in Murakami City. Bordering Yamagata Prefecture and known as the "Fortress of the North," what were the residents focused on? At the end of August, at Murakami General Hospital in Murakami City, a tense atmosphere filled the room. ■Dr. Shimura "How soon will the ambulance arrive?" ■Emergency Medical Technician "It looks like they'll be arriving in 10 minutes." Resident Yuji Shimura. He was scheduled for internal medicine training, but was suddenly called to the emergency room. At the time, both the doctor helicopter and the ambulance were on their way at the same time. ■ Murakami General Hospital, Masashi Shimura "We will conduct a thorough examination due to fever and suspected heart failure and pneumonia." The man was brought in from Awashima. There are no doctors here normally. Shimura had just returned from a two-week residency. Currently, six medical residents are studying at Murakami General Hospital. He was also seen in the pediatrics department. His name was Ryotaro Sugimoto. ■ Murakami General Hospital, Ryotaro Sugimoto "How are you?" Only a few days remain of his two-month pediatric training. ■ Murakami General Hospital, Ryotaro Sugimoto "Did he have a cough and runny nose, but no fever? Is his runny nose clear? Is it greenish?" Shinya Tsukano watches over Sugimoto as he asks multiple questions. After the examination, he reflects. ■ Dr. Shinya Tsukano, Murakami General Hospital "He's doing well. He's healthy. He's smiling, so he's fine. For now, he's either bronchitis or just a runny nose." A native of Niigata, Dr. Tsukano has been dedicated to the field for 40 years, focusing on pediatrics. ■ Dr. Shinya Tsukano, Murakami General Hospital "When they get better, they leave the hospital with a smile on their face. Pediatricians are truly happy when they see patients leave in good health." Dr. Tsukano retired from Niigata City Hospital in March. In April, he joined Murakami General Hospital, where the pediatrics department was facing a crisis following the retirement of a full-time doctor. He is now 66 years old. There's something he's conscious of. ■ Dr. Shinya Tsukano, Murakami General Hospital "He's getting older, so it's important to nurture the next generation." Initial training after graduating from medical school is two years. Mr. Sugimoto spent his first year training in his hometown of Osaka. He's noticed a difference since coming to Murakami. ■Murakami General Hospital, Ryotaro Sugimoto "We don't see many seriously ill children here, so instead, we have them come in multiple times as outpatients, and we monitor each area." Sugimoto also spent two weeks training on Awashima this summer. ■Murakami General Hospital, Ryotaro Sugimoto "This clinic doesn't offer blood tests, so we really do examine patients. We examine their bodies, listen to their sounds, and ask questions about how long the symptoms have been present, what their daily lives are like, and how accurately we can listen to information. I really felt that conversational skills are also important." One evening after a break in the clinic, The residents gathered together. Director Soichi Sugitani had assigned them a report on their Awashima training. ■Murakami General Hospital, Director Soichi Sugitani "It was really fun. Reading this really gave me a sense of what life is like on Awashima. It's a shame you're experiencing so many interesting things and not sharing them." The residents' supervisor, Daisuke Ugamura, is in his 12th year. ■Murakami General Hospital, Dr. Daisuke Ugamura "The biggest challenge and difficulty in providing medical care on a remote island is the lack of equipment and other resources. This is where we need to utilize the on-site response capabilities we've cultivated." The report details an incident on the island. Director Sugitani called it "a treasure trove." ■Murakami General Hospital, Director Soichi Sugitani "If we create an emergency manual for remote islands, we could be the first in Japan to share it. If you work in a high-volume (large) hospital in the city, you probably won't get bitten by a centipede." ■Murakami General Hospital, Ryoutaro Sugimoto "Should we have it used on other remote islands, or should we use it as feedback for Awashima?" ■Murakami General Hospital, Director Soichi Sugitani "There's no point in sharing it unless it benefits the local community first."
