Why? Emerging research suggests that the quality of the deep spinal muscles, particularly the multifidus, may influence postoperative pain and functional outcomes. Studies have shown that higher levels of preoperative multifidus fatty infiltration are associated with less favourable recovery trajectories after lumbar surgery. This does not mean surgery doesn’t work. It does not mean imaging dictates destiny. And it certainly does not mean your spine is fragile. It does suggest that preparation matters. The spine is strong. Most back pain is not dangerous. But optimisation matters. There are multiple professionals (surgeons, physiotherapists, chiropractors, osteopaths and more) who treat back and spine problems. But often they work independent of each other and few patients have someone taking responsibility of overall oversight of care in a team.
