When Is a Headache Dangerous? A Neurologist Explains the Warning Signs
Most headaches do not need imaging. A few patterns do. The difference between the two is the difference between an anxious patient and a delayed diagnosis. Know the patterns. Get evaluated in time. This video is for anyone who has wondered whether their headaches need a scan. For anyone whose family doctor has reassured them too quickly, or imaged them too freely. For anyone who knows something has changed and cannot get the words across. We walk through the SNOOP framework that emergency physicians and neurologists use at the bedside: Systemic features (fever, cancer, immunocompromise, weight loss), Neurologic signs (focal weakness, papilledema, altered consciousness, seizure), Onset sudden (the thunderclap headache), Older onset (a new headache after fifty), and Pattern change (a headache pattern that has shifted). We also cover the red flags beyond SNOOP: headaches that wake you from sleep, positional headaches, Valsalva-precipitated headaches, headaches in pregnancy or postpartum, and post-trauma headaches that get worse. We close with the workup itself. CT urgently when onset is sudden. MRI for everything else. Lumbar puncture when subarachnoid hemorrhage is suspected and CT is negative. Targeted labs when the picture demands. We tell Karen's story. Karen is a fifty-eight-year-old retired teacher who had no history of headaches until three months before her visit.
