LAA Closure Evidence Update (2/2): OPTION & CLOSURE-AF Interpretation

How do OPTION and CLOSURE-AF change our view on LAA closure? In this second part of my Cardiology Update 2025 lecture from London (Dec 8–9, 2025), I focus on the most recent randomized data: OPTION Trial: WATCHMAN FLX vs. oral anticoagulation after AF ablation Study design, endpoints and 3-year results Non-inferiority for efficacy and superiority for non-procedural bleeding CLOSURE-AF: High-risk, frail AF patients with increased bleeding risk Different population (older age, higher CHA₂DS₂-VASc and HAS-BLED, CKD, prior bleeding, multimorbidity) Higher early procedural complication rates and impact on the composite endpoint Why OPTION and CLOSURE-AF cannot be directly compared, and why they address different patient populations (post-ablation vs. very frail HBR patients) Practical take-home messages and outlook (CHAMPION AF, LAAOS IV concepts). All comments on CLOSURE-AF in this talk are based on the data that were available at the time of the presentation and may change when full datasets and publications become accessible.
