Transforming Care Through PEN-Plus: Tanzania's PEN-Plus Story tells the story of how Tanzania is expanding access to life-saving care for people living with severe noncommunicable diseases through the PEN-Plus model. By bringing specialized services closer to communities and equipping nurses and clinical officers to manage complex conditions such as type 1 diabetes, sickle cell disease, and rheumatic and congenital heart disease, Tanzania is demonstrating that quality care can be delivered at district and regional hospitals—even in resource-constrained settings. Through the voices of patients, families, healthcare workers, and health leaders, the documentary follows the journey of hope, resilience, and transformation made possible by monthly PEN-Plus clinic days. It captures how regular, integrated care is helping children return to school, adults return to work, and families look to the future with renewed confidence.
