PCOS TREATMENT The treatment for polycystic ovary syndrome differs significantly between women who are not currently trying to conceive and those who are. In women who are not trying to get pregnant, treatment will be aimed at reversing or alleviating symptoms, such as hirsutism and all those derived from hypoandrogenism, obesity, or oligomenorrhea. However, in women trying to conceive, the priority will be to improve fertility. WOMEN NOT SEEKING PREGNANCY In women not seeking pregnancy, the main objectives will be: To improve the symptoms of hyperandrogenism: hirsutism, acne, hair loss… To correct menstrual irregularities such as oligomenorrhea (which is sometimes accompanied by heavy periods) or amenorrhea. To prevent endometrial hyperplasia and the possibility of developing endometrial cancer as a consequence of chronic anovulation. To improve metabolic abnormalities related to PCOS: obesity, insulin resistance, metabolic syndrome… 1. LIFESTYLE CHANGES: Lifestyle changes, such as promoting physical exercise and weight loss, have proven effective in reducing insulin resistance and hyperandrogenism. 2. HORMONAL CONTRACEPTIVES: Combined oral contraceptives usually improve hyperandrogenism and its related symptoms. 3. OTHER: Metformin, antiandrogens, GnRH analogs, cosmetic treatments... WOMEN TRYING TO CONCEIVE For women trying to conceive, the priority is to improve fertility. 1. LIFESTYLE CHANGES: In obese women, the first step will be to recommend weight loss. Again, relatively modest weight loss (5-10%) improves metabolic status and decreases androgen levels. 2. OVULATION INDUCERS Clomiphene has been used for years as an ovulation inducer. However, Letrozole appears to be more effective in achieving ovulation and improves the pregnancy rate. Both drugs act as antiestrogens. By decreasing estrogenic feedback on the pituitary gland, they increase FSH levels, thus favoring the selection of a dominant follicle and ovulation. 3. OTHER TREATMENTS: Metformin, gonadotropin therapy, laparoscopic surgery...
