The premutation occurs, as we've already discussed, when the number of repeats in this gene ranges from 55 to 200. If the number of repeats falls within this range, then in about a quarter of cases—not every woman, but about a quarter—women experience premature ovarian failure. The standard clinical picture we observe is that menarche occurs absolutely on time, meaning menstruation begins at a completely normal age, and then becomes established and occurs quite regularly. But at some point, they may begin to become confused, meaning there may be delays in menstruation or irregular cycles. Typically, if such a young woman thinks about fertility early, she can become pregnant and give birth before the first symptoms of diminished ovarian reserve appear. However, with a fairly high probability, typically somewhere between the ages of 29 and 40, a woman with a premutation in the FMR1 gene often experiences premature menopause, and she becomes infertile. At our Center, we try to treat such patients, attempting to reverse this situation temporarily with specialized therapy.
