GKV-Stabilisierungsgesetz – so kann’s nicht funktionieren! | Laura Dalhaus zum aktuellen Stand

The Statutory Health Insurance Stabilization Act was passed by the cabinet – but there is considerable resistance to it. Dr. Laura Dalhaus therefore puts the current developments in the healthcare system into perspective. Dissatisfaction is growing: One in three people is now dissatisfied with the healthcare system. Even Nina Warken apparently would have preferred a different solution. At the same time, the federal government is further withdrawing from financing statutory health insurance – the federal subsidy is to be reduced from 14.5 to 12.5 billion euros. Laura believes that many decisions were made under enormous time pressure, according to the motto: "The main thing is to present results." Calculations by the Handelsblatt newspaper further call the austerity plans into question. One problem remains the financing of health insurance contributions for recipients of basic income support. These are borne solely by those with statutory health insurance, while those with private insurance and civil servants do not contribute. At the same time, the pharmaceutical industry is hardly being held accountable. Laura explains this using the example of Jardiance, an SGLT2 inhibitor, whose price has risen dramatically since its approval for kidney failure. Children and adolescents are particularly affected. Here, cuts are becoming increasingly severe – for example, through the closure of the Dietrich Bonhoeffer Clinic for young people with addictions. Laura warns against further weakening the outpatient sector. If outpatient structures collapse, the shift towards outpatient care will also fail, even though many inpatient services could be provided on an outpatient basis. The Health Finance Commission has now proposed an emergency plan, including a sugar tax. This tax is indeed included in the draft legislation – but not until 2028. At the same time, the reality of healthcare provision remains strained. Medical associations are accused of exaggeration, while health insurance companies are demanding mandatory appointment pools for specialists. In North Rhine-Westphalia, large parts of the emergency dispatch centers are also slated for closure, a move that Hendrik Wüst has already sharply criticized. Laura also finds the discussion surrounding the salaries of health insurance executives contradictory: A cap is rejected as an infringement on self-governance, while at the same time, self-employed physicians are facing ever-increasing regulation. Furthermore, there are too many health insurance companies in Germany – digitalization and AI could significantly alleviate this burden, in her view. Laura also points to the integrated care model (HZV), which provides care to around 11 million insured individuals through a voluntary primary care physician system. Scientific data shows that this model improves healthcare provision. She strongly criticizes the fact that health insurance companies are simultaneously blocking this system in some cases. She is also critical of the planned emergency care reform. Doctors are expected to provide 24/7 emergency services while simultaneously running their practices. Given the staff shortage, Laura finds it unclear how this is supposed to work. The consequences are already evident: Almost half of all practices in Berlin are considering selling their practices early, and many are actively planning this step. The staff shortage is exacerbating the situation, especially in large cities. Laura is convinced that health policy can only function if the training and continuing education of healthcare professionals is consistently integrated into the process.
