British SAR mission in fog
Can you spot any of the 9 hazardous attitudes or clues the crew could have picked up on that a link in an error chain may be forming? How about 1. Invulnerability, Complacency, Denial. 2. Macho "Can Do" Attitude, 3. Resignation (on the part of the crew member going down on the wire and the copilot floating the bubbles and not making an assertive statement) 4. Not Willing to challenge 'Experts', (uncomfortable but not being assertive) 5. Press-on-itis (Must get to the patient not matter what) 6. Risky Shift (Groups can make riskier decisions than an individual) Anchoring Bias
