It tests a candidate’s ability to think critically, prioritize patient needs, and make safe clinical decisions in realistic patient scenarios. Because each choice affects the direction of the case, success requires a systematic approach to assessment, treatment, and reassessment. Several high-yield clinical situations are important to recognize. Severe hyperkalemia requires immediate treatment, especially when EKG changes such as peaked T waves, widened QRS complexes, or bradycardia are present. Treatment may include calcium gluconate, insulin with dextrose, albuterol, sodium bicarbonate when acidosis is present, and potassium removal with diuretics or dialysis. Ventilated patients may develop auto-PEEP when incomplete exhalation causes air trapping. This can increase peak pressures, worsen hypercapnia, and reduce venous return. Management includes lowering the respiratory rate, reducing tidal volume if needed, increasing expiratory time, and using bronchodilators for obstruction. Ventilator asynchrony should also be managed by adjusting flow, trigger sensitivity, inspiratory time, or ventilator mode before relying on sedation. The CSE also requires accurate interpretation of common disease patterns. Cardiogenic pulmonary edema is associated with heart failure, elevated BNP, high PCWP, and cardiomegaly, while noncardiogenic pulmonary edema such as ARDS presents with diffuse infiltrates and normal cardiac pressures. Hypercapnic respiratory failure requires support based on severity, ranging from medication adjustment to BiPAP or intubation. Other key topics include accidental extubation, capnography interpretation, COPD phenotypes, asthma management, and bronchiectasis. Candidates should know when to provide oxygen, initiate noninvasive ventilation, reintubate, confirm airway placement, treat asthma exacerbations, and promote airway clearance. Overall, passing the CSE depends on staying calm, gathering relevant information, avoiding unnecessary actions, and choosing interventions that match the patient’s condition.
