It tests your ability to think critically, prioritize patient needs, and make sound clinical decisions in real time. Unlike traditional multiple-choice exams, the CSE presents evolving patient scenarios where each decision affects the outcome. Correct, timely choices move the case forward, while unnecessary or incorrect actions can reduce your score. Success requires understanding the exam structure and applying a systematic approach. The CSE includes 20 scored cases and 2 unscored pretest cases, with each scenario alternating between Information Gathering (IG) and Decision Making (DM). Scoring ranges from positive to negative points, so efficiency and accuracy are essential. A key strategy is prioritizing life-threatening conditions. If a patient shows signs of respiratory or cardiopulmonary failure, immediate intervention takes precedence over further assessment. Delaying treatment can result in lost points and missed opportunities. During the IG phase, avoid over-ordering tests. Each diagnostic choice should have a clear clinical purpose. A structured approach starting with basic bedside assessments and progressing to more advanced tests helps prevent unnecessary selections. Recognizing common disease patterns such as COPD exacerbations, pulmonary embolism, and ARDS allows for faster, more accurate decisions. Strong knowledge of respiratory care management is also critical. This includes adjusting ventilator settings based on blood gas results, identifying when to initiate weaning, and distinguishing when to use noninvasive ventilation versus intubation. Oxygen therapy must be tailored to the severity of hypoxemia and the patient’s condition. Additionally, familiarity with neonatal and pediatric care, as well as proper use of CPAP and BiPAP, is essential.
