I specialize in Geriatric Neuropsychiatry from Unifesp. I have a PhD in Health Sciences from USP. In this video, we'll discuss why urinary incontinence is more common in the elderly and how we should manage it. First, what is urinary incontinence? Urinary incontinence is characterized by the involuntary loss of urine through the urethra, significantly impairing quality of life in old age. Why is it more common in the elderly? As we age, the bladder muscles become more sensitive and weak, resulting in a tendency for urine to leak. Furthermore, with the most common chronic diseases in old age, most seniors take medications that contribute to urinary incontinence, including diuretics that increase urine volume. One of the factors that can also be linked to male urinary incontinence is an enlarged prostate, whether due to benign prostatic hyperplasia or even a disease that enlarges the prostate, such as cancer. Female urinary incontinence can be caused by menopause due to decreased estrogen levels. Another factor contributing to urinary incontinence in the elderly is the weakening of the sphincter, the muscle that acts to hold urine from escaping through the urethra. This muscle eventually loses its elasticity, contributing to urinary incontinence in the elderly. What are the consequences of urinary incontinence? Social isolation, low self-esteem, and other factors. How can we guide our seniors? First, understand that there is treatment for this condition and seek help from a healthcare professional to identify the cause, which can be varied in the elderly. Exercises for urinary incontinence strengthen the pelvic muscles. Regarding the use of diuretics, it is important to discuss dosage and the real need for the medication with your doctor, or even the possibility of switching, and as a last resort, surgical treatment for urinary incontinence.
