How to properly rinse your nose to prevent acute otitis? I often see patients who are convinced that rinsing their nose can cause an ear infection – #otitis. Nasal irrigation is one of the most important treatments for a runny nose. In the middle of the head, there's a tube-shaped structure that connects the nose to the ear. Specifically, the middle ear, which is inside us. What is a #runny nose #rhinitis? It's an inflammatory process in the nasal cavity, like any inflammatory process—it's accompanied by swelling and the formation of mucus (exudate). When there's mucus and swelling in the nose, the mucus can't drain normally and stagnates in the nasal cavity, increasing the risk of it being backed up into the eustachian tube. Furthermore, if you try to blow your nose, due to the swelling, the mucus will have difficulty exiting and can also back up into the eustachian tube. However, the swelling itself also closes the eustachian tube, causing the ear cavity to become airtight, creating negative pressure. This also causes pathological processes in the tympanic cavity itself, accompanied by the formation of transudate—fluid in the tympanic cavity. In other words, it doesn't flow into the tympanic cavity; it forms there. Why and how should you properly irrigate your nose? First, if you have acute inflammation accompanied by swelling and nasal congestion, you need to reduce the swelling before rinsing, otherwise the irrigation solution simply won't reach the nasal cavity. Therefore, 15 minutes before rinsing, you should spray a vasoconstrictor. All swelling is reduced, and the nasal cavity is clear. Then you can irrigate your nose. If we irrigate with a spray, like a shower, the stream breaks against the walls of the nasal cavity and doesn't create enough pressure to push the mucus, as some of my colleagues say, into the Eustachian tube. It will simply be a trickle of solution flowing along the bottom of the nasal cavity. And it's unlikely to flow into the Eustachian tube. Why would it flow into a small crevice when there's a wide space in front—the choana? These are simple laws of physics. The second option is rinsing with a solution. In this case, the solution will also flow down the nasal passage, and if you lean forward properly, it will flow into the other nasal passage and out the other side. In any case, if you tilt your head to the side, there's a risk that the solution will enter the Eustachian tube and through it, enter the middle ear. Even if you're rinsing your nose with an antiseptic solution, this can cause otitis media. Therefore, if you want to avoid otitis media, avoid tilting your head to the side when rinsing your nose. Next, to prevent otitis media, you need to blow your nose correctly. If your nose is very swollen, or if you pinch your nostrils with your fingers when blowing, then if you encounter resistance, a stream of air containing particles of inflammatory exudate (mucus) can enter both the Eustachian tube and the ear. Therefore, as I said, it's essential to reduce swelling with a decongestant spray and blow your nose properly—one nostril at a time, using smooth, gentle movements. In fact, from an evidence-based perspective, there are no studies proving a link between the development of acute otitis media and nasal irrigation. Cases where patients developed otitis media despite nasal irrigation are either the result of a complication of a runny nose unrelated to the irrigation. This means the patient would have developed otitis media even without the irrigation. Or, the errors I mentioned above were made during the irrigation.
