Clinical Perspectives on Refractory Shock Management: Vasopressor Strategy, Perfusion, & Escalation

Despite adherence to evidence based protocols, septic shock frequently evolves in ways that defy expectations. This expert-led presentation is designed for experienced critical care clinicians seeking deeper insight into how shock pathophysiology, perfusion, and vasopressor responsiveness change as patients progress beyond routine management. Through case based discussion and moderated expert dialogue, faculty explores how seasoned clinicians recognize key inflection points, assess and reassess the physiologic and pathophysiologic factors when perfusion targets are unmet, and weigh the risks and benefits of vasopressor options and strategies. Presenters: Richard G. Wunderink, MD David W. Boldt, MD, MS Samir S. Awad, MD, MPH, FACS This session was organized by Innoviva (https://www.inva.com) and presented during the Society of Critical Care Medicine’s (SCCM) 2026 Critical Care Congress.
