From Detection to Treatment: Prostate Cancer Theranostics and the Future of Precision Oncology

Hofman, MBBS is Nuclear Medicine Physician and Clinician-Scientist at Peter MacCallum Cancer Centre; Lead for Peter Mac’s PET/CT program and Prostate Cancer Theranostics and Imaging Centre of Excellence. This lecture traces the evolution of PSMA PET and PSMA-targeted theranostics in prostate cancer, from early imaging with sodium fluoride and fluorocholine to the rapid clinical impact of PSMA PET. The talk reviews key evidence from the proPSMA, PRIMARY2, TheraP, and VISION trials, highlighting improved staging accuracy, biopsy selection, patient stratification, treatment response, and quality-of-life benefits. It also explores the future of theranostics, including total-body PET, dosimetry, FDG/PSMA-based patient selection, combination therapies, new radionuclides, and the long-term goal of moving radiopharmaceutical therapy toward more personalized and potentially curative treatment.
