Why NOT radiation for prostate cancer?

Radiation therapy is one of the most effective treatments for localized prostate cancer—but it is not the right choice for every patient. In this video, I discuss situations where I may recommend against radiation therapy and explain why treatment decisions should always be individualized. These are not absolute contraindications, but rather factors that make radiation less attractive and surgery, active surveillance, or another approach worth considering. Topics include: Severe urinary symptoms (high IPSS, enlarged prostate / BPH, urinary retention) Difficulty maintaining a full bladder or empty rectum Patients who strongly prefer surgical removal of the prostate Erectile function considerations Inflammatory bowel disease and connective tissue disorders Prior pelvic radiation Widely metastatic disease Active surveillance and observation Other practical considerations discussed during consultation Using a patient case, I walk through how these factors influence treatment recommendations and demonstrate why there is rarely a "one-size-fits-all" answer. My goal is not to convince every patient to receive radiation therapy. My goal is to help patients understand which treatment is most appropriate for them.
