Conoid ligament- medial (insert about 4.5 cm from end of clavicle) Trapezoid ligament- lateral (insert about 3 cm from end of clavicle) Common fracture Usually fall on the shoulder or sometimes on an out stretched hand. Commonest site- Middle 1/3rd (80-85%) Lateral 1/3rd (2nd most common 10-15%) Medial 1/3rd (rarest) Usually displaced- Distal fragment displaces medially and downwards (weight of arm, gravity and pull by pectoralis major muscle) Medial fragment displace upwards (pull by sterno-cleidomastoid muscle) Examination- Check for deformity and tenting of the skin May be an impending open fracture Watch for distracted clavicle May have scapulo-thoracic dissociation Check for brachial plexus injury & neurovascular bundle (subclavian vessels about 1cm from clavicle) X-ray- Get xray of both shoulders (Bilateral views to measure the shortening). In X-ray look for- shortening Displacement Comminution Z-type fracture Risk factors for nonunion- Displacement and comminution Older female Smokers Fracture of lateral 1/3rd of clavicle with displacement of medial fragment Treatment- Fracture of the clavicle unite readily even if displaced Reduction of fragment- not essential Triangular sling ( for 4-6 weeks) Active shoulder exercises (10-14 days after injury) Figure of 8 bandage Open reduction and internal fixation
