Shoulder Dislocation Hindi | sign and symptoms diagnosis and treatment | shoulder dislocation treatment without surgery | Shoulder pain | Dislocation of Shoulder • Most common joint in human body to dislocate • Commonly in adults & rare in children • Anterior dislocation more common Mechanism • Fall on outstretched hand with shoulder abducted and externally rotated • Direct force pushing humerus head out of glenoid cavity • Note- most commonly direct blow on the front of the shoulder- Posterior shoulder dislocation Factors responsible for shoulder dislocation • The shallowness of glenoid cavity • Extra ordinary range of movement • Underlying condition like ligamentous laxity (hypermobile joints that are very flexible and have a wider range of motion than most people), glenoid dysplasia • Vulnerability of the joint during stressful activities of the upper limb Classification 1. Anterior dislocation- 3 subtype Preglenoid Sub-coracoid Sub-clavicular/ infraclavicular 2. Posterior dislocation 3. Inferior dislocation (Luxatio erecta) Diagnosis Presenting complaint • Patient enters in casualty with shoulder abducted and elbow supported with other hand. • History of fall on an outstretched hand followed by pain and inability to move the shoulder. 1. Dugas test- • Patient seated. Instruct him to touch the opposite shoulder and bring the elbow to the chest wall. • Positive Test: Inability to touch the opposite shoulder because of pain indicates anterior dislocation of the humeral head. 2. Hamilton ruler test • A ruler can be placed between the acromion and the lateral epicondyle. • Normally, this is not possible because of the contour of the deltoid muscle. • Hamilton's ruler sign is positive when the tip of the acromion may be joined to the lateral epicondyle of the humerus with a straight line.
