CLASSIFICATION Primary dysmenorrhea: Recurrent cramp-like abdominal pain in the hypogastric region that occurs during menstruation in the absence of any objective pathology. This is a diagnosis of exclusion. It is more common in adolescents and young women. Secondary dysmenorrhea: the same pain, but explained by the presence of pathologies such as endometriosis, adenomyosis, or uterine fibroids. EPIDEMIOLOGY Menstrual pain affects 50-90% of women of reproductive age. There are risk factors and protective factors that we will discuss in the video. PATHOGENESIS Prostaglandins play an important role in the pathogenesis of dysmenorrhea. DIAGNOSIS The diagnosis is primarily clinical. Primary dysmenorrhea should be a diagnosis of exclusion: an underlying pathology (genital tract disorders, PID, endometriosis, adenomyosis, fibroids). TREATMENT OF PRIMARY DYSMENORRHEA The goal is to reduce menstrual pain and improve quality of life. Once other pathologies have been ruled out, empirical treatment can be initiated. Non-pharmacological recommendations: exercise, dry heat... Medical treatment: NSAIDs, paracetamol, or hormonal contraceptives. Second-line treatment: electrical stimulation, GnRH analogues, surgical treatments... Alternative therapies: hypnosis, acupuncture, behavioral therapy, dietary changes, vitamin supplements...
