We knock you out without knowing how.
Why don't we know how anaesthesia works? It switches off consciousness by a route science still can't fully explain. Every day, all over the world, we switch a person's consciousness off, cut them open, and switch it back on. General anaesthesia is one of the most routine miracles in medicine, and here is the unsettling part: we still cannot fully explain how it works. The mystery starts with the drugs themselves. The chemicals that can render you unconscious are wildly different from one another, gases, alcohols, steroids, molecules that share almost no obvious structure. That is very strange, because normally a drug works by fitting one specific target like a key in a lock, and these keys look nothing alike. An old idea called the Meyer Overton correlation noticed that how well a substance dissolves in fat predicts how strong an anaesthetic it is, which hinted the drugs might act broadly on the fatty membranes of nerve cells. The modern view is more specific: they seem to latch onto certain protein channels on neurons, especially ones that quieten brain activity. But the deepest gap is not about single cells, it is about consciousness itself. The leading idea is that anaesthesia does not simply shut the brain down, individual regions can stay quite active, it disconnects them, breaking the long range communication that lets the brain integrate information into a unified experience, with the thalamus acting as a kind of central switch. And that is exactly why the puzzle is so hard. To fully explain how anaesthesia removes consciousness, we would first need to explain what consciousness is, and that remains one of the biggest open questions in all of science.
