What is a kidney nodule? A kidney nodule is a change usually seen on ultrasound, CT scan, or MRI. And not every nodule is cancer! In fact, most small findings turn out to be benign or very slow-growing lesions. A very common example is simple cysts, which are simply fluid-filled sacs and, in the vast majority of cases, pose no risk. 2. The Impact of Size and Characteristics The size of the nodule and its imaging characteristics are the main factors: • Less than 4 cm: Many cases can be monitored only with periodic examinations, as long as there are no suspicious signs. • Over 4 cm: The chance of malignancy increases, and surgery is often considered. In addition to size, we also consider whether the nodule captures contrast on CT scans, whether it has calcifications, and whether it is solid or cystic. All of this is part of risk stratification, as recommended by the AUA (American Urological Association) and EAU (European Association of Urology) guidelines. 3. Most Common Types of Kidney Lesions • Simple cysts: Extremely common and generally do not require treatment. • Complex cysts: Analyzed using the Bosniak classification, which ranges from type I to IV. The higher the category, the greater the risk of malignancy. • For example: Bosniak I and II are benign, • Bosniak IIF require monitoring, • Bosniak III and IV are higher risk and often require surgery. • Angiomyolipoma: a benign tumor composed of blood vessels, fat, and muscle, but which can grow and cause bleeding. • Renal cell carcinoma: the most common kidney cancer, often discovered incidentally during tests performed for another reason. 4. How is the correct diagnosis made? Ultrasound is only the first step. The test of choice is usually a contrast-enhanced CT scan or MRI, which help define the nature of the nodule. In some selected cases, a kidney biopsy may be indicated. According to the AUA, a biopsy can help differentiate benign from malignant lesions, avoiding unnecessary surgery for tumors that pose no risk. 5. What symptoms may appear? In most cases, the patient feels nothing. The finding is incidental. However, when symptoms do occur, they may include: • Blood in the urine (hematuria) • Persistent low back pain • Palpable abdominal mass These symptoms, however, are rare in the early stages. 6. Treatment – ​​What do the guidelines say? Treatment depends on the size, location, and risk of the lesion: • Active monitoring (Active surveillance): indicated for small nodules, usually less than 2 cm, and in patients with other diseases or advanced age. • Partial surgery (partial nephrectomy): when it is possible to remove only the nodule, preserving the remainder of the kidney. This is the preferred recommendation, whenever technically feasible, according to the EAU 2024. • Radical surgery (total nephrectomy): indicated for large lesions or when it is not possible to preserve the kidney. • Minimally invasive techniques, such as cryoablation or radiofrequency, may be considered in selected cases, especially for patients who are not clinically fit for surgery.
