In this type of surgery, most of the nasal structures are truly preserved. Most modern preservation techniques are different. They are mainly focused on preserving the osseocartilaginous connection of the dorsum - in surgical language, the keystone area, or K-area, the roof of the nose. This helps preserve the natural dorsal lines and polygons. But at the same time, many other parts of the nose may still be changed significantly. A good example is the famous let-down technique. First, the side walls of the nasal bones are mobilized. Then, a portion of the septum inside the nose is removed. After that, the entire dorsum is lowered as one unit. And then the surgeon may still change the angle, position, length, and shape of the tip cartilages and stabilize them with sutures. So what was actually preserved? Mainly the roof. That is why preservation rhinoplasty should not be seen as magic. It is simply a surgical concept with specific indications. So do not look only at the name of the technique. Look at the result.
