Spine surgery can be very effective — but only in specific situations. One of the most common misunderstandings is that surgery is a reliable solution for back pain itself. In reality, most spine operations are designed to relieve pressure on nerves or the spinal cord, improving leg symptoms and protecting neurological function. So when does surgery actually help back pain? In this video, I explain: • Why most spine surgery is aimed at nerve-related symptoms • Why back pain alone is often not a surgical problem • The specific situations where surgery can help back pain • The role of conditions such as spondylolisthesis and instability • Why diagnosis and patient selection are critical • How to think about surgical decision-making more clearly The key message is not that surgery doesn’t work — but that it works best when used for the right reasons. The spine is strong. Most back pain is not dangerous. And careful diagnosis matters more than intervention. There are multiple professionals (surgeons, physiotherapists, chiropractors, osteopaths and more) who treat back and spine problems. But often they work independent of each other and few patients have someone taking responsibility of overall oversight of care in a team. This is why I founded The Spine Multi-Disciplinary team (MDT), to bring all professionals of spine care together under my oversight as a surgeon to provide spine care that gets results.
