When I think of this kind of Yankee-style game, Kunio-kun comes to mind first (personally), but Ka•Ge•Ki was also one of my favorite action games. I played it a lot at the arcade, but I never managed to beat it... This Mega Drive version is a little easier, with items like health recovery. Thanks to that, I was able to complete it. You can also punch forward or backward, but they don't hit very well (and you're more likely to take damage), so the best strategy is to punch sideways. If you maintain a good sense of distance from your opponent, your punches will almost never miss, so I think you can make it to the end with any character fighting the same way.
