Continuing the previous discussion, in this video, Dr. Risma Kaban, SpA(K) explains how to manage lactation for mothers with premature babies, based on the baby's oromotor skills and digestive function. Whenever possible, fresh expressed breast milk should be given to premature babies, as it contains better antibodies than refrigerated expressed breast milk. If a premature baby is forced to receive donor breast milk, the donor must meet several requirements, such as the baby's age being less than 6 months, the baby's milk needs being sufficient, there being no history of infectious diseases, and several other requirements, which can be seen in this video. The donated breast milk must also be cultured to check for the presence of bacteria. In essence, the administration of donor breast milk is inherently very strict. Given that premature babies have significant micronutrient needs (protein and minerals), which are often not met by breast milk alone (especially babies weighing under 1,500 grams), they often need to be supplemented with Human Milk Fortifier (HMF), which can help accelerate bone growth and mineralization. Just like full-term babies, it's also very important to regularly plot (record and monitor) the growth charts of premature babies. For premature babies, the Fenton chart is used until 40 weeks of gestation and a body length of 45 cm. After that, the WHO curve can be used. Strive to keep your baby within the 50th percentile.
