Many people repeatedly try to stretch their seemingly "shortened" hamstrings. The problem: Often, the restricted movement isn't primarily due to the muscle itself, but rather to an irritated sciatic nerve or nerve root – often, but not always, in connection with a herniated disc. In this video, I explain why neural irritation can feel like a shortened hamstring and why intensive stretching can sometimes even worsen the symptoms. For the sake of clarity: In the video, I use the term "straight leg raise test" for simplicity. This refers not only to the classic straight leg raise, but also to the neurodynamic examination, including extensions and additional tests such as the slump test, the Neri sign (achieved by flexing the cervical spine), and the Bragard sign (achieved by dorsiflexing the foot). In practice, these tests are often combined directly – therefore, for me personally, it's "one test" – clinically useful, but not scientifically rigorous, I'm aware of that. Therefore, my statement regarding specificity doesn't apply universally to the classic straight leg raise alone. In some studies, the slump test shows high sensitivity and moderate to good specificity for lumbar disc herniations. The straight leg raise is often sensitive, but less specific depending on the study. The crucial factor is always the combination of a thorough medical history, neurological examination, spinal examination, and several appropriate tests. So, importantly: Not every pulling sensation in the back of the thigh means the muscle is shortened. If the mobility of the back of the thigh is significantly altered by the position of the spine, head, or foot, this could indicate that the nerve is limiting the movement, rather than the muscle. In that case, further aggressive stretching isn't automatically the best solution, but could potentially be the cause of your symptoms! So, be careful! : ...
