While providing available treatment and rehabilitation, we also take practical measures and accept the condition to prevent the vicious cycle of medication discontinuation due to difficulties in daily life, leading to relapse. Schizophrenia is a mental illness characterized by symptoms such as auditory hallucinations and delusions in the acute phase. It is believed to be primarily caused by excessive dopamine activity in the brain, and is not uncommon, affecting approximately 1 in 100 people. Schizophrenia symptoms can be broadly divided into three categories. These symptoms include "positive symptoms," such as auditory hallucinations and delusions, which manifest as experiences not experienced when healthy; "negative symptoms," such as a loss of motivation and reduced emotional expression, which are inherently present; and "cognitive dysfunction," which causes difficulty with attention and concentration. The course of the disease can be divided into four stages: the prodromal phase (a precursor to insomnia and anxiety), the acute phase (a period when auditory hallucinations and delusions are intense), the respite phase (a period when the acute phase subsides but negative symptoms are prominent), and the recovery phase (a period when activity gradually returns and the patient aims to reintegrate into society). (2) Fatigue and its Impact in Schizophrenia Even after drug treatment has helped patients overcome the acute phase and positive symptoms such as auditory hallucinations and delusions have improved, this is not the end of the story. Even after recovery, negative symptoms such as decreased motivation and fatigue continue to predominate. This invisible hardship often poses a major obstacle to daily life and reintegration into society. Three difficulties are particularly prominent during the respite phase. Symptoms include a lack of motivation or energy for anything, spending the whole day in a daze, a lack of emotions and an inability to enjoy even fun activities, and a near-depletion of both physical and mental energy, followed by rapid fatigue. In fact, reports suggest that approximately 60% of people with schizophrenia experience fatigue to the point that it interferes with their daily lives. This severe fatigue significantly impedes daily life, reducing activity and lowering quality of life. Furthermore, if fatigue makes medication management difficult, the risk of relapse increases. Why do people with schizophrenia tire easily? First, negative symptoms impair the brain's ability to generate motivation and enthusiasm, resulting in a persistent lack of energy. Cognitive impairment requires the brain to work harder to accomplish tasks that were previously easy, leading to mental fatigue. Furthermore, even if symptoms improve, continuing to deal with lingering auditory hallucinations and anxiety can be physically draining. Fatigue can create a vicious cycle. It can make basic daily activities like bathing and meal preparation difficult, leading to reversal of day-night cycles, disrupted daily rhythms, and irregular eating habits. Furthermore, if fatigue makes it difficult to properly manage medication, it can trigger a relapse. (3) How to Deal with Fatigue in Schizophrenia Rehabilitation alone has its limits when it comes to dealing with fatigue. It's important to accept the symptoms and make practical efforts to address them. Treatment can be broadly divided into three categories. 1) Treatment and Rehabilitation Antipsychotics are extremely important in the treatment of schizophrenia, but their effectiveness in addressing negative symptoms like fatigue is limited. This is where rehabilitation becomes important. Negative symptoms can be alleviated by carefully progressing rehabilitation with the aim of improving cognitive func...
