Emphysème pulmonaire: traitement de l'emphysème
Emphysema treatment: Pulmonary emphysema treatment largely involves reducing risk factors and managing associated diseases. Among the therapeutic approaches, we will look at prevention, drug treatments in the chronic phase and during exacerbations, surgical treatments, and rehabilitation. The most important step in preventing emphysema and slowing its progression is to quit smoking. Since smoking is a major factor in the onset of emphysema, quitting smoking helps reduce mortality. Another important measure in emphysema prevention is to avoid inhaling substances that can damage the lungs, such as air pollution. There are also pharmacological emphysema treatments available, such as bronchodilators. Bronchodilators are actually a general term that includes several classes of drugs, but all of which share the effect of dilating the airways. The first class is beta-2 agonists, which stimulate beta receptors. The second class of bronchodilators is anticholinergics, most of which end with the suffix -opium. Leukotriene inhibitors can also reduce airway inflammation, as leukotrienes play an important role in the cascade that ultimately leads to inflammation. Inhaled corticosteroids can also be administered to a person with emphysema, as part of the treatment of an exacerbation or attack of emphysema. These are painful periods when symptoms are more severe and significant. Therefore, administering corticosteroids, which are anti-inflammatory drugs, will reduce the pain caused by the inflammation associated with emphysema. Other drug treatments include: Antibiotics Oxygen therapy Vaccines For surgical treatments, lung volume reduction surgery is a recognized therapeutic option in the management of patients with severe and disabling pulmonary emphysema. Lung transplant for emphysema: Lung transplantation can also be performed in severe emphysema, when the lungs are extremely damaged, and the patient has exhausted emphysema treatment. An innovative minimally invasive technology proposed in the treatment of severe emphysema is PneumRX. The principle is to insert metallic devices (nitinol devices) to attempt to reduce the volume of the lungs and restore their elasticity. The steps are as follows: first, you must determine the position of the fiberscope at the segment of the lobe to be treated. Next, you'll place a guide wire, which has a blunt tip so it can't damage the lung. You'll locate the area you want to treat. Using a catheter, you'll insert these coils into the position you've determined. The coil actually has a shape memory, meaning that when you insert it, it's in a longitudinal position. When you're in the optimal position, the chosen position at the segment, you'll gradually withdraw the catheter, and the coil will return to its original shape, which is a spiral, making one or two turns on itself, which will allow it to grip and achieve the effect of retraction and lung elasticity.
