Hello, everyone with pain. This is PAIN LAB :) People in their 20s might be a bit confused, but those over 30 have probably experienced arm numbness at least once, right? As we all know, when we think of arm numbness, we often think of cervical disc herniation, and cervical disc herniation often involves arm numbness. We also think of cervical disc herniation as a symptom of numbness caused by the disc compressing the nerves. As I've repeatedly mentioned in my videos, not everyone with a compressed disc experiences numbness. If you're wondering what I mean, please refer to this video. If your neck condition worsens, you may experience pain that extends to your neck, shoulders, and arms. This can be accompanied by numbness, paresthesia, and muscle weakness. You may also experience a tingling sensation on the inside of your shoulder blade. If your symptoms are severe enough, you may be diagnosed with cervical disc herniation and receive a nerve block injection. However, while cervical disc herniation should result in some degree of symptom improvement, many patients report no improvement at all. As you may know, cervical disc herniation isn't the only cause of these arm and hand symptoms. "Scallop syndrome" can also present with symptoms very similar to cervical disc herniation. If you experience numbness in your arm, but no abnormalities were found on X-rays, MRIs, or CT scans, and the doctor incorrectly attributed it to a forward head posture or straight neck, or even if you underwent treatment for narrowing of the cervical vertebrae, the condition is likely caused by scalene syndrome. Of course, these two conditions can be differentiated through careful questioning, palpation, and a physical examination. Cervical disc herniation and scalene syndrome have very subtle differences in symptoms, so it can be confusing to diagnose them solely based on the patient's reported symptoms. This is why we need to perform various movements and even perform a physical examination. Let's take a closer look at the scalene muscle. The scalene muscle is divided into three parts: the anterior, middle, and posterior scalene muscles, as shown in the picture. It tilts the head to one side and acts as an accessory respiratory muscle related to breathing. This video also explains when the scalene muscle is uncomfortable. Did you watch the video? I mentioned that if the scalene muscle is tense, side bending is difficult. The cause of scalene muscle syndrome is that the scalene muscle is responsible for the sides of the cervical spine, so if the muscles are strained to the sides, it can be caused by sleeping in an incorrect position, such as sleeping with your head tilted to one side or sleeping with your head tilted to one side. Also, if the scalene muscle becomes tense due to hunching, or if the scalene muscle is shortened due to thoracic breathing or incorrect breathing, nerve compression can occur. Nerves from the cervical spine pass through the scalene muscle. Nerves and blood vessels pass between the anterior and middle scalene muscles. When these muscles tense, they can pinch these nerves and blood vessels, causing numbness, paresthesia, and muscle weakness in the arms. You may also wake up with swollen, cold hands. Scale syndrome is often associated with thoracic outlet syndrome, which puts a lot of pressure on the median and ulnar nerves. Be sure to watch this video. Scale syndrome also affects the dorsal scapular and long thoracic nerves, which originate from the scalene muscle, so it can cause severe pain on the inner side of the scapula. To find the middle scalene muscle, consider it the exact center of your neck when viewed from the side. It's where the anterior and posterior borders of the upper and lower halves of your neck meet. That's where the middle scalene muscle is. Press firmly on the middle scapula and move your head left and right. This may help relieve symptoms. Moving slightly forward from here is the anterior scalene muscle, and back from here is the posterior scalene muscle, so move them slightly to release them. Remember the nerve we mentioned earlier? Applying too much pressure can cause nerve damage, so apply just the right amount. You need to apply just the right amount of pressure. If you've tried this nerve release technique on your own and your symptoms don't go away, or if they're worsening, please seek immediate medical attention. For scalene syndrome, acupuncture and moxibustion can quickly improve the condition. However, if thoracic outlet syndrome has become chronic, herbal medicine may be necessary.
