At least 201 babies and nine mothers could have lived if their maternity care had been better. That is the conclusion of an independent report into repeated failures at the Shrewsbury and Telford Hospital NHS Trust in Shropshire. It found a culture where mistakes were not investigated, risks were not properly assessed and parents were not listened to.
