Nerve pathway: The femoral nerve originates between vertebrae L2, L3, and L4 and descends along the front of the thigh to the inner side of the knee. The obturator nerve (L2-L4) also innervates the knee joint internally. When these nerve roots are compressed by a herniated disc, protrusion, or chronic lumbar muscle tension, the brain interprets the signal as knee pain—even if the knee is structurally sound. Muscular pathway: The iliopsoas, quadriceps (rectus femoris), hamstring, and tensor fasciae latae muscles directly connect the lumbar spine and pelvis to the knee. When they are shortened or unbalanced, they alter knee alignment, compress the patella, and cause asymmetrical cartilage wear. Gluteal imbalance: The gluteus medius is the central stabilizer of the hip. When weak, the pelvis drops laterally with each step, simultaneously overloading the lumbar spine and knee. It is the most underestimated link in this chain. ❓ FREQUENTLY ASKED QUESTIONS 🤔 Can knee pain be caused by lower back pain even without back pain? Yes. Referred pain—originating in the spine but felt in the knee—can occur without the person feeling any lower back pain. The nerve transmits the signal directly to the knee. This is called referred pain of vertebral origin. 🤔 How do I know if my knee pain is from my knee or lower back? Some signs that suggest a lumbar origin: pain that changes intensity depending on the position of the spine, pain that is not related to impact or strain on the knee, pain accompanied by tingling or numbness in the thigh or leg, worsening with prolonged sitting. 🤔 Can I do these exercises if I have osteoarthritis in my knee? Yes, with adaptations. Strengthening the gluteus and stretching the psoas are especially indicated for knee osteoarthritis, as they reduce the inappropriate load on the joint. Avoid movements that cause acute local pain in the knee. 🤔 Does a herniated disc in the lumbar region always cause knee pain? Not always. It depends on the level of the herniation and which nerve root is being compressed. Herniated discs at L2, L3, and L4 are more likely to cause pain on the front or inside of the knee. 🤔 Do these exercises replace physical therapy? They are an excellent starting point and a powerful complement. For cases with neurological impairment, intense acute pain, or post-surgery, individualized monitoring is essential. 🤔 How often should I do these exercises? Ideally, 4 to 5 times a week for consistent results. Strengthening the gluteus medius can be done daily as it is low-impact. 🤔 I have gonarthrosis (osteoarthritis of the knee). Do these exercises help? Yes.
