The fracture mechanism is longitudinal compression of the talus/talus against the distal surface of the tibia. It can also be associated with rotational movement. The type of fracture depends on the rotation applied and the position of the foot at the time of the trauma. In the past, these fractures were the result of low-energy trauma, such as minor falls. Currently, pilon fractures are more common due to high-energy trauma, such as traffic accidents or sports injuries. They can be associated with fractures of the talus, calcaneus, tibial plateau, pelvis, acetabulum, and spine. These fractures were previously treated using various methods. Currently, the trend is to achieve open reduction and stable internal fixation with a plate and screws, reconstructing the tibial articular surface and applying a supporting bone graft to the tibial metaphysis. Many authors have reported good results with internal fixation, with good bone reduction, reconstruction of the articular surface, internal stabilization, and early movement. Unfortunately, these good results are hindered by the poor vascularity of the region, which is further impaired by local traumatic damage. Another negative factor is that these fractures are usually exposed and comminuted, with significant joint deformation associated with bone loss and fibular fracture. Tibial Pilon Fracture The tibial pilon is the end portion of the main bone of the leg, the tibia. It is the region that contains the tibial surface that makes up the ankle joint, along with the fibula (formerly called the peroneus) and the talus (foot bone). Tibial pilon fractures are generally caused by severe trauma, such as motorcycle falls, car accidents, being run over, etc. In most cases, the tibial pilon bone is fragmented into small pieces and needs to be reconstructed as best as possible to minimize post-traumatic effects. Tibial pilon fractures almost always require surgical treatment. However, in more severe cases, the skin surrounding the tibial pilon region becomes severely damaged, preventing definitive treatment in a single surgical procedure. Recent medical studies show that staged treatment of these fractures (in at least two separate surgical steps) yields better results. Because these are complex fractures that require careful surgical planning, additional tests, such as computed tomography, are highly recommended whenever possible. This so-called "staged treatment" consists of applying an external fixator that stabilizes and aligns the fracture until the skin and soft tissues surrounding the bone are in optimal condition for definitive surgery. Definitive surgery (the second stage of treatment) should be planned based on imaging tests (x-rays and CT scans) and consists of reconstructing the fractured bone and internal fixation with plates and screws. Recovery time after tibial pilon surgery varies depending on the severity of the injury. In the best-case scenario, recovery takes 3 to 6 months. Unfortunately, most patients who suffer this type of fracture experience some type of permanent after-effect, including frequent swelling, limited ankle movement, pain with physical exertion, etc.
