The lower leg is composed of two long bones: the fibula (or fibula) and the tibia. The fibula is located on the outer, lower side of the leg and articulates with the tibia at the knee and ankle, and with the talus further down. tibia, lateral malleolus During certain activities, such as landing from a jump and especially ankle sprains, considerable force is applied to the lateral (outer) malleolus. When this trauma is very strong, the external malleolus can rupture. The fracture can affect the elderly, but also young people. Often, a lateral malleolus rupture occurs in conjunction with an ankle sprain or another fracture of the foot, ankle, or leg. There are many types of lateral malleolus fractures: avulsion, stress, Pott's fracture, compound, displaced, incomplete, comminuted, etc. Bimalleolar and Trimalleolar Fractures Bimalleolar means bilateral, meaning both the fibular and tibial malleolus of the ankle are broken. A trimalleolar fracture means that the external and internal malleolus are damaged, and a third posterior tibial malleolus is also broken. In most cases of bimalleolar and trimalleolar fractures, the ankle is unstable. In this case, the inner ankle ligaments are also damaged. An ultrasound is recommended to determine if the medial ligaments are injured. A bimalleolar fracture may occur simultaneously with a dislocation. Associated Injuries Injury to the ankle joint surface Tibiofibular syndesmosis fracture Lateral talar process tear Fracture at the base of the fifth metatarsal bone Causes of Lateral Malleolus Fractures A fracture of the fibular malleolus often occurs during an ankle sprain, especially if the force applied is significant. It can also occur from landing after a jump, trauma, or a collision with another player in sports. Signs and Symptoms of a Lateral Malleolus Fracture Patients with this injury experience sharp, sudden pain on the outside of the ankle when the trauma occurs. A ruptured malleolus causes the patient to limp because they are unable to put weight on the injured leg. The discomfort is most intense at night or in the morning when waking up. A fracture of the lateral malleolus causes swelling, bruising, and tenderness to the touch in the affected area of the bone. The pain increases with some ankle movements. Diagnosis of a Lateral Malleolus Fracture A subjective and objective medical examination is essential for diagnosing a malleolus fracture. fracture, spirals, x-ray, ankle, fibula, external, lateral, leg, pain, injury, inflammation X-ray of a lateral malleolus fracture Other evaluations, such as MRI, CT scan, or bone scan, may be necessary in some cases to assess the condition of the soft tissues and identify a stress fracture that does not appear on x-rays. Treatment Non-surgical treatment for an isolated lateral malleolus fracture If the ankle is stable, surgery may not be necessary, that is, if the fracture is non-displaced or only displaced. The type of treatment needed depends on the point at which the bone is broken. There are several ways to protect the fracture while the bone heals. Some doctors recommend a brace or short cast to support weight and immobilize the ankle joint. Some orthopedists advise putting weight on the broken leg immediately, while others advise waiting 6 weeks. Non-surgical Treatment for Bimalleolar and Trimalleolar Fractures These injuries are considered unstable, and your doctor will usually recommend surgery. Non-surgical treatment may be considered if you have serious health problems, if the risk of surgery is too great, or if you are unable to walk.
