The REAL Pilot Mistake That Got Dana Killed!

Free → https://pilotdebrief.com/pattern On a June afternoon in 2016, a Cirrus SR20 entered the traffic pattern at one of Houston's busiest Class B airports, sharing final approach with a steady stream of Boeing 737s. Three approaches and three go-arounds later, the airplane spun into a parking lot half a mile north of the runway. The pilot was flying into William P. Hobby Airport on June 9, 2016, with two passengers aboard. Hobby is a busy Class B environment dominated by airline traffic, and from the moment she entered the pattern, the controller began issuing a barrage of instructions — multiple runway changes, sequencing calls, and tight spacing between her Cirrus and the 737s converging on the field. Her first landing attempt was broken off when an air carrier overtook her. Her second was waved off because she was too high to land safely. On the third approach, with a new local controller now on frequency, she again struggled to get down in time and elected to go around herself. As she initiated the go-around in a left turn, the new controller transmitted a lengthy, complex clearance. That is where the airplane came apart. The Cirrus SR20's published go-around procedure calls for 81 to 83 knots indicated airspeed before retracting the flaps. Recovered data showed the pilot raised full flaps at 58 knots while in a turn — well below safe airspeed. The wing exceeded its critical angle of attack, the airplane departed in an accelerated stall and spin, and impacted terrain. The pilot and both passengers were killed. The NTSB found the pilot's improper go-around procedure — failing to ensure a safe airspeed before raising the flaps — was the probable cause, with the controllers' decision to keep her in the pattern, the unnecessarily complex clearance issued during a critical phase of flight, and the pilot's own lack of assertiveness as contributing factors. The pattern here is a workload trap. She was trying to comply with everything ATC asked of her, and every instruction was consistent with her goal of landing at Hobby. But compliance is not the same as command. A pilot-in-command can request resequencing, can tell the controller to stand by, or can simply say "unable."
