What exactly is endometriosis? This question concerns many, not only patients but also doctors, partners, and relatives. What exactly is endometriosis? Lower abdominal pain, infertility, menstrual problems, and many other symptoms. Of course, endometriosis can also be completely symptom-free. Sometimes you notice it after a long time because you can't get pregnant despite constantly trying. Endometriosis is a chronic disease. Because it is hormone-dependent, symptoms do not appear before the first menstrual period and improve after menopause. Endometriosis affects approximately 2 to 10 percent of all women, making it a common disease. However, due to its extensive and nonspecific symptoms, it is sometimes never diagnosed and often only after years, making it difficult to accurately assess its prevalence. Women between the ages of 20 and 40 are particularly affected, generally meaning women of childbearing age. In addition, endometriosis tends to run in families. And there are so-called risk factors: 1) Genetic predisposition (i.e., a familial predisposition to the disease), 2) short menstrual cycles, 3) an early first menstrual period, the so-called "menarche," and a late menopause, and 4) a late first pregnancy. What is endometriosis? Endometriosis is a disease in which cells SIMILAR (!) to the endometrium settle in certain areas, e.g., outside the uterus. This can, in principle, occur anywhere in the body. However, it most commonly occurs 1) in the fallopian tubes, 2) the ovaries, 3) the intestines, 4) the bladder, 5) the peritoneum, 6) outside the abdominal cavity (i.e., 'extra-abdominal'), such as in the lungs. These cell accumulations are called endometrial lesions and, like the lining of the uterus, follow the hormonal cycle. However, unlike the endometrium, cell debris and blood from the endometrial lesions cannot be excreted regularly. Sometimes the body can break them down unnoticed, but sometimes not. If this is not the case, pain can occur due to 1) inflammation, 2) cyst formation, or 3) adhesions, the intensity of which can vary.
