Cervical pain is one of the leading causes of physical disability worldwide and is frequently associated with sleep quality problems and insomnia. Let's explore the medical literature to better understand the potential benefits of using a pillow in the treatment of cervical pain. Let's start with a basic question: is it better to sleep with or without a pillow? For patients with cervical pain, we already have more than 30 scientific studies demonstrating that those who sleep with a pillow have lower pain levels and intensity compared to patients who sleep without a pillow. But this leads us to another question: should we then sleep with a low pillow or a high pillow? To answer this question, let's look at the data from two studies that evaluated this issue. The first study evaluated the level of trapezius muscle contracture in patients who slept with a low pillow, a neutral pillow, and a high pillow. It was observed that patients who slept with a neutral pillow maintained a more relaxed trapezius muscle, with the worst situation being those who slept with a low pillow. The second study evaluated, through radiographs of the cervical spine, the maintenance of the normal cervical curvature (cervical lordosis) in patients who did not use pillows, in patients with 10 cm high pillows, and in patients with 20 cm high pillows. The researchers were able to demonstrate that patients who slept with 10 cm (average height) pillows were those who presented greater preservation of the usual neck curvature, the cervical lordosis. This suggests that these patients would have less overload on the joints and also on the associated musculature. Another very frequent question concerns the pillow material. We are bombarded with pillow advertisements every day, featuring a wide variety of materials and technologies. But are pillows made of one material necessarily better than others? To answer this question, let's look at a study that compared the performance of synthetic pillows, made of latex, with pillows made of goose down. This study demonstrated that synthetic pillows tend to lose less of their physical properties compared to goose down pillows, maintaining their height and benefiting the natural alignment of the cervical spine during sleep. But what about patients suffering from neck pain? Would using a more "suitable" pillow be beneficial in helping to resolve the pain? Another study recruited several patients with neck pain and offered them pillows that kept them in the most neutral position possible, without any other type of treatment or medication. With just these simple adjustments, neck pain was reduced by about 30%. So, what we have today in the scientific literature regarding neck pain and pillows (although we still lack better studies) indicates that the use of medium pillows, which favor the physiological curvature of the spine and help reduce overload in the cervical region, can be beneficial for patients suffering from neck pain. It is worth noting that it is not only the pillow that should be adjusted, as neck pain has several causes, and its treatment should be multifactorial. For example, insomnia and sleep disorders can be as important as, or even more important than, the pillow itself. Therefore, it is important to have a good sleep pattern and, of course, if a suitable pillow contributes to this, even better. Stress is also a major contributing factor to neck pain. We have studies that measured trapezius muscle contracture during periods of stress compared to non-stressful periods, and they showed that stress does indeed worsen neck pain. Therefore, in summary, adjusting your pillow can be helpful, but it's far from the only solution to employ.
