Laura's Vorschläge zur Rettung des Gesundheitssystems
Laura's proposals for saving the healthcare system have sparked much discussion. The head of DAK (a German health insurance company) called for suggestions – and Laura came up with 22. To explain them better, here is the episode: 1. End over- and under-provision in orthodontics Treatments should be strictly based on medical necessity and not on financial incentives. The goal is to avoid unnecessary procedures and reduce costs. 2. Eliminate reimbursement for cannabis flowers Since cannabis is legally available, Laura sees no reason for statutory health insurance to cover the costs. Medical services should be limited to necessary and evidence-based therapies. 3. Reduce fees for cataract surgery The reimbursement for these procedures is currently disproportionately high. Adjusting it would reduce financial imbalances in the system. 4. Abolish skin cancer screening Screening has not been proven to reduce mortality. Without a clear benefit, this service should no longer be funded. Nevertheless, anyone can, of course, go to the doctor if they feel unsure. 5. More strictly regulate drug costs More focus on discount contracts and cost-benefit analyses. The goal is to reduce expensive and ineffective medications. 6. Finance basic income recipients with tax revenue The costs should not be borne solely by those with statutory health insurance. This is a task for society as a whole. 7. Increase taxes on sugar, alcohol, and tobacco Unhealthy consumer goods should be taxed more heavily, while healthy foods should become more affordable. This creates incentives for prevention. 8. Abolish Disease Management Programs (DMPs) Many programs do not demonstrate a clear benefit for patients. Instead, individualized, needs-based care should be provided. 9. Shift the outpatient system to outpatient care Outpatient services belong in doctors' offices, not hospitals. This is more efficient and cost-effective. 10. End billing fraud in hospitals Perverse incentives such as unnecessary hospital admissions should be stopped. Transparent and honest billing is essential. 11. Overcome sector boundaries Outpatient services should be reimbursed equally everywhere – regardless of whether they take place in a hospital or a doctor's office. 12. Level 1 i-Hospitals – Create small outpatient care centers Short-term treatments (e.g., infusions) should be possible without hospitalization. This relieves the burden on hospitals and improves care. 13. Eliminate statutory health insurance benefits Services without an evidence-based foundation (e.g., homeopathy) should no longer be funded. Focus on medically appropriate care. 14. Introduce the polluter-pays principle Unhealthy lifestyles could lead to higher premiums. This should create incentives for health-conscious behavior. 15. Positive List for Statutory Health Insurance Benefits Clear definition of which services are covered. Only evidence-based medicine should be part of the care provided. 16. Reintroduce Quarterly Reports & Limit Doctor Visits Fewer repeat visits and more efficient patient management. No-shows should also have consequences. 17. Digitization and Capping of Administrative Costs Administrative processes should be digitized and costs limited. The goal is more efficient use of resources. 18. Limit Additional Benefits for Health Insurance Employees Benefits and additional perks should be reduced. Savings potential lies in the administrative system. 19. Abolish Health Insurance Advertising Since all health insurance funds offer similar services, advertising is unnecessary. This saves considerable costs. 20. Reduce Office and Building Costs More working from home and fewer expensive locations could lead to significant savings. 21. Reduce auditing and legal procedures (Blue Ocean Strategy) Complex procedures should only be carried out for significant sums. This saves time and money. 22. Reduce overall bureaucracy Many processes cost more than they yield. Simplification would make the system more efficient.
