Free → https://pilotdebrief.com/pattern On a clear June morning in 2019, a Beechcraft King Air lifted off from Dillingham Airfield on Oahu's North Shore with a pilot and ten skydivers aboard. Seconds later, it was inverted inside the airport fence. The aircraft, operated by the Oahu Parachute Center, was departing in day VFR conditions for a routine jump run. Eleven people were on board a twin-turboprop that, on paper, should have handled the climb without issue. It didn't. During initial climb, the King Air rolled, lost control, and impacted the ground inside the airport perimeter, erupting in a post-crash fire that killed everyone on board. The NTSB investigation pulled at threads that ran far deeper than a single bad takeoff. Investigators found an aggressive rotation that resulted in an accelerated stall at an altitude too low to recover from. They also found an airplane operating near its aft center-of-gravity limit, a pilot who hadn't been trained for the handling qualities he encountered there, and a left wing that had been twisted by a prior tail-strike event and never properly repaired — quietly eroding the stall margin every flight. The NTSB further cited the operator and its contract mechanic for failing to keep the airplane airworthy, and pointed at the FAA's insufficient regulatory framework for overseeing Part 91 parachute jump operations, including a lack of awareness that the pilot's flight instructor had been providing substandard training. This is the pattern: an airplane that wasn't what it looked like, a pilot who didn't know what he didn't know, an operator that wasn't catching either, and a regulatory gap big enough for all of it to live inside.
