About the episode: When we think of the very beginning of a pregnancy, we visualize the feat of ovulation, the incredible ascent of the sperm, the meeting of the two, and then, like the magic of life, it's on, the pregnancy is launched! Except there is one stage that is of utmost importance and determines whether or not the resulting embryo will be able to continue its development: implantation in the mother's endometrium. And this is where a subtle immune balance is established, to accept this half-foreign being. In this episode, I was fortunate to welcome Dr. Nathalie Ledée, a gynecologist specializing in fertility and immunologist, and founder of Matrice Lab, who has been studying this immune response that allows implantation, and its potential dysfunctions, for years. She explains why the endometrium accepts, or sometimes rejects, an embryo, how maternal immunity works—neither too much nor too little—and how the embryo itself collaborates to be accepted. We also discuss tests to assess possible immune overactivity or underactivity, which can explain implantation failures or early pregnancy loss, and ways to overcome them.
